Please review the Student-Athlete Participation Forms below. You may email your completed forms to your coach or bring them to campus when you arrive. We do not accept IMAGES of the forms. Please be sure to scan and email PDF attachments during the submission process.
Sports physicals must be completed on the Boyce College Physical form found below. We will not accept physicals completed on other forms.
All forms and paperwork MUST be completed before a student-athlete can participate in any athletic practice, training, or meeting.
